
Housing Stability Programs
Clients enrolled in our Housing Stability Programs are offered longer-term supportive services, working diligently with a community social worker to obtain and maintain independent housing.
Our staff use evidence-based practices and trauma-informed care to provide external supports such as advocacy and financial counseling. Clients also engage in goal planning, individualized counseling, and are provided community resource referrals.
Pathways to MyHome
Pathways to My Home is a rapid rehousing program that provides comprehensive and efficiency-based case management and financial assistance to identify client barriers to housing. Pathways uses a strengths-based approach to secure permanent housing for families and individuals.
Our Programs
Permanent Supportive Housing
Home2Stay offers permanent supportive housing for individuals and families that include at least one member with a disability and who face chronic homelessness. This program integrates housing assistance with intensive case management, and supportive services to ensure long-term stability in housing.
Bridge House
Bridge House provides intensive case management aimed at assisting residents in obtaining permanent housing, including temporary residency in one of our four Bridge House units from six to nine months. This initiative delivers immediate support for individuals facing a housing crisis, creating a clear pathway to stability and minimizing the length of homelessness.
Transitional Housing
The Homestead Housing Program provides long-term transitional housing for residents of Lancaster County who are considered a vulnerable population within the homelessness sector. This includes clients facing youth homelessness (18 to 21 years old), senior homelessness (55+) or a single parent household with a minor dependent(s). The program manages 6 units and aims to achieve permanent housing within a year; with an additional available extension, offering extensive support and personalized case management to each participant.
